When you  complete the application , scan it and email it back to us at Barnessafetyservicesinc@gmail.com or take a picture front and back of the completed  application and send it by text to 709-631-2509(bring the completed form with you on the course start date

When we receive your application, we will reach out and confirm the payment method you want to use to complete the booking
